For Applicants for Certificate of No Pending Case, the following are the requirements:
- Accomplished Request for Certificate of No Pending Administrative Case Form;
- Valid ID and a Certified Photocopy of the valid ID of the requesting party. If request is through a representative, include authorization letter and valid ID of the representative;
- Proof of payment for the P100 fee. The fee of P100.00 shall be paid in the Regional Office upon presentation of the valid IDs.
- Attached is the Application Form with a directive to accomplish the same then scan e-mail to our Office at firstname.lastname@example.org or email@example.com
Note: For Retirement purposes, there is no need to secure a Certificate of No Pending Case from this Regional Office. Instead, the retiring employees will simply fill up and submit the “Declaration of Pendency/Non-Pendency of Case” to their respective agencies. Attached is a copy of a “Declaration of Pendency/Non-Pendency of Case”(Annex “A”).
