GSIS Umid Number, SSS Number, PhilHealth Number, Tax Identification Number, HDMF/PAG-IBIG Number, Divers License Number, Passport Number, PRC License Number, Voter’s Number, Postal Number, Police Clearance Number, School/Student Number, Company/Employee Number
- Educational Background:
Schools attended, degree/course, date attended, honors/commendations received, diploma/certificate of graduation, transcript of records/copy of grades
- Employment Background:
Name of agency or office, address, contact number, position or job title, dates of employment, performance ratings, trainings or seminars attended
- Health information: Information about pregnancy or physical disabilities, if any.
- Where applicable, conviction of any administrative or criminal offenses.
- References: Name, address, telephone number
- Other information which may be indicated in the Applicant’s Personal Data Sheet (PDS), including but not limited to, other license or eligibilities, membership in an organization, affiliation.
We may obtain these personal data from the forms provided for the processing of the following online transactions, together with the required supporting documents, other written records, and photographic images:
- Request for Eligibility under Special Laws (BHW, BNS, BOE, SME, EDP, HGE, CSC MC No. 11, s. 1996, as amended (Category II), FSHGE)
- Request for Authentication of Certificate of Eligibility
- Request for Correction of Personal Information in the Records of the Commission
- Request for Issuance of Certificate of No Pending Administrative Case (CONPAC)
- Application for Distance Learning Program
- Request for Authentication of Document(s)
- Application for CSC Administered Examinations (CSE-PPT, BCLTE, ICLTE, BFP, BJMP, FSOE, EOPT, Promotional)
DATA SHARING
We may share your personal data with the Civil Service Commission Central Office, and/or other CSC Regional and/or Field Offices, in case of requests for assistance, verification, report submission, and conduct of administrative investigation. We may also share the collected data for the purpose of complying with court orders, subpoenas and/or other legal obligations.
PERSONAL DATA SECURITY
This Office has put in place physical, technical, and managerial procedures designed to help prevent unauthorized access, to maintain data security, and to use correctly the information we collect online. While we use such means and methods to protect the data, we cannot guarantee its absolute security.
