If GCQ/MGCQ:
  1. Download the Sanggunian Member Eligibility (SME) form here.
  2. Please see the requirements on the 2nd page/back of the form.
  3. Access the link for Online Booking: https://csccaragaesd.setmore.com/.
  4. Bring with you the complete supporting documents.
If ECQ/MECQ:
  1. Download the Sanggunian Member Eligibility (SME) form here.
  2. Please see the requirements on the 2nd page/back of the form.
  3. Scan the accomplished Sanggunian Member Eligibility Form and supporting documents and email to cscrocaraga@gmail.com.
  4. Once received, the ESD staff will acknowledge your application for initial verification and processing
  5. You will be notified of the schedule of release/pick-up of the requested certificate and pay the required fee/s at the counter at the clients’ receiving area.
  6. On pick-up bring printed notice email and any valid ID and follow CSC’s In Office Transaction Protocol.
  7. In the CSC RO, proceed to the client’s receiving area and present the accomplished SME Form and pay 500.00 pesos per copy
  8. Wait for your name to be called.
