INSTRUCTIONS:

1. Fill up the verification slip form;
2. Attach Personal Data Sheet (PDS) and Certificate of Eligibility (CoE).
3. Scan and email the accomplished verification slip form, PDS and CoE to cscrocaraga@gmail.com.
4. Mail or send through courier the original documents to Civil Service Commission Caraga Regional Office, Doongan Road, Butuan City.
5. We will email and send thru post the result of the verification.

ADVISORY:

This is to advise our partner agencies that requests for VERIFICATION OF ELIGIBILITIES shall be the responsibility of the Agency HRMO/HRMO Designate and not of the individual/employee concerned guided by the following information:

Requests for verification of Certificate of Eligibility outside Caraga Region shall also be facilitated, but with a longer processing time as examination records are still to be requested from the region having jurisdiction over the matter. 

This Advisory will supersede the previously issued Advisory in February 2021.
